LIPA Companion Guides.

 The Companion Guides supplied by LIPA were developed in cooperation with the Oregon HIPAA Forum. Oregon DHS/OMAP used the same base guides to produce the Companion Guides needed to submit fee-for-service claims directly to the state.  LIPA followed the recommendations made by the Oregon HIPAA Forum to produce guides that closely match the guides you will receive from other payers in Oregon.

 The guides are available as online HTML pages, a detailed PDF file and a short PDF file.  The detailed PDF contains most of the required fields and all the notes from LIPA for the data required to submit to LIPA.  The short PDF contains an abbreviated guide with just the fields that LIPA has special usage notes. 

 These Companion Guides are not a definitive listing of all required fields.  When testing you may find additional information is required for your specialty.  You must include this required data regardless of whether or not LIPA uses this information for adjudication.

 LIPA currently only accepts transactions using the 4010A1 format.  This is the addenda version published earlier this year (2003) in the Federal Register.
